Guaranteed Growth Annuity (GGA) forms
Annuity claims are paid after the Company ensures:
- You are the true beneficiary
- The contract is current and in force
- That all conditions of the contract have been met
Required documents
When filing a claim, these documents are necessary:
- Certified copy of death certificate
- GGA Claim to Annuity Benefits (Required for all beneficiaries listed on the contract)
- Any additional claim form that is required for the payment option you have selected (see additional option forms below)
Additional documents
Not all options are available to all beneficiaries. Depending on your personal situation, these additional documents may be necessary:
- GGA Settlement Option Election for Annuity Benefits Available when a beneficiary wishes to annuitize the death benefit under a payment option
- GGA Lump Sum Distribution Form Available when receiving a lump sum distribution in the form of a check or an Equitable Access account
- GGA Successor Owner/Annuitant (SOA) Option Form Available when the spouse wants to continue the existing contract in their name and take over ownership
- Beneficiary Guide: To assist with important decisions after the loss of a loved one
